How to Obtain Transcripts of Credits

• Your complete name, address, telephone number and/or e-mail address, year of graduation or year(s) of studies and your signature (If you enrolled under a different name, please provide that name.)

• The name and address of the person(s) to whom you wish the transcript(s) to be sent
